    Health Requirements
    All incoming 6th grade students are required to submit a completed school physical and updated immunizations. Please note that some of these immunizations are different from the requirements for elementary school. These immunizations include: Tdap, 2 doses of Varicella (chicken pox), a completed Hepatitis B series (3 doses), 2 doses of MMR, TD series, and a Meningococcal vaccine.

    Completed physical forms may be turned in to the school health office as soon as they are completed or mailed to Hadley Junior High Attn: Nurse. 

    If your child will be new to Hadley Junior High School as a 7th or 8th grader, the health requirements are the same as if your child would be entering as a 6th grader. Please see the health requirements listed above.

    Dental Requirements
    A dental examination is also required for students entering 6th grade. The deadline for dental examination forms is May 15th of the current school year. We will accept dental exams that are completed up to 18 months prior to the due date. For the 2020-2021 school year, we will accept dental exams completed between November 15th, 2020 and May 15th, 2022. Please click HERE to the Dental Examination Form

    Health Conditions and Medications at School 

    If your child requires medication during the school day, please keep in mind that a school medication administration form needs to be completed by your child’s physician before any medication can be administered at school. Please turn this form in along with the medication, sealed in it’s original packaging, with your child’s name on it. For the safety of all our students, students are not permitted to self carry any medications, unless emergency medication such as epinephrine or an asthma inhaler. All medications are to be kept in the nurse’s office. Please see below for the appropriate forms.

    Food Allergies
    If your child has a food or other severe allergy that requires the use of epinephrine, please have an allergy action plan completed and signed by your child’s physician along with a completed medication administration form.  Please see below for the allergy action plan.

    If your child has asthma and requires an inhaler at school, please be sure to inform the school nurse if your child will be self-carrying his or her inhaler. Additionally, we ask that your child’s doctor completes an asthma action plan.  Below is a link for the asthma questionnaire that we ask all parents to complete annually for their child. 

    If your child has a seizure disorder, please see below for a seizure action plan to be completed each school year. If your child requires emergency medication at school, please also have a medication administration form completed. 
    If your child is a diabetic, insulin or non-insulin dependent, please have your child’s endocrinologist provide a diabetes medical management plan for school. It is the responsibility of the parent to provide all necessary testing supplies and treatments (such as snacks, glucose tabs, and glucagon) for use during the school day. We will happily store these items in the nurse’s office.

    PE and Interscholastic Sports
    All students who plan to participate in interscholastic sports for the current school year will need to have an up-to-date physical on file in the nurse's office in order to be able to participate in practice. We will accept sports physical forms OR the child health examination forms, however they will only be valid for ONE YEAR from their completion date. Please note that only one form is needed, not both. Please contact the school nurse to see if your child has a valid physical on file.
    If your child has sustained an injury that will prevent him or her from participating in PE or Sports,  please provide a medical note from a treating physician listing these restrictions. We will be more than happy to make the appropriate accommodations for your child.

    As summer comes to an end and the school year begins, also comes the arrival of cold and flu season. At the start of each school year, it is not uncommon for us to see an increase in flu and respiratory illnesses.  Please keep the following preventative measures in mind to help keep our students, staff, and community safe and healthy:

    • Wash your hands often with soap and water, for at least 20 seconds. This is especially important after you’ve coughed, sneezed, or blown your nose, before and after eating, and anytime after coming into contact with someone sick.
    • Avoid touching your eyes, nose and mouth
    • Cover your mouth when you cough or sneeze. Do your best to cough or sneeze into a tissue or the elbow of your sleeve
    • Avoid close contact with others who are sick
    • Clean and disinfect frequently touched objects like your cellphone, laptop, desks, and door handles.
    • Most importantly, STAY HOME if you are sick.

    In efforts to keep our students and staff safe and healthy, we ask that if your child has a fever, flu-like symptoms, or has vomiting/diarrhea, they should NOT be in school until they are fever/symptom free for at least 24 hours, without needing medication. Please do not send your child to school if they are ill. When calling in an absence for your child, kindly let the office know if your child is being kept home due to fevers or flu-like symptoms.
