Military Family Data

Periodically the Illinois State Board of Education (ISBE) or other state and/or federal agencies request that we collect various demographic data regarding our students.

This request for military active duty is voluntary. If you are currently active in the military and wish to provide your information, please complete this form. If your military status does change during the school year we request that you contact your child's school office with the updated information.

You may print the Military Family Data form and submit to your child's school.
