Physical Examination and Immunizations

Glen Ellyn School District 41 policy requires a child entering kindergarten or 6th grade or NEW to District 41 have a current State of Illinois school physical and updated immunization record on file PRIOR to October 15, 2024 in order to avoid exclusion.

New Students who enter school AFTER October 15, 2024 have 30 calendar days to provide the health office with a current State of Illinois school physical and proof of immunizations in order to avoid exclusion.

Physicals

A State of Illinois school physical examination is required for entrance into Preschool, Kindergarten, 6th grade, and/or when transferring from an out of state school. The health history portion of the physical examination form must be completed and signed by the parent in order for the physical to be valid. The exam must be dated within the past twelve months of the student's first day of attendance. 

Physical examinations may be acquired through your primary medical provider. If you do not have a primary medical provider, you may contact your school nurse or the DuPage County Health Department for referrals to local health clinics that provide such services.

The physical exam must be documented on the Illinois Certificate of Child Health Exam. The form is available from the nurse's office, on the Illinois Department of Human Services website, or from your physician's office. All sections of the physical exam form, including the parent history section must be completed.

All physicals are due in the nurse's office on or before October 15th of that school year. New students, kindergarten and 6th grade students, who do not have a physical and immunization record on file with the school by October 15th of that school year will not be allowed to attend school.

Students who are new to the District, and begin after October 15, will have 30 days from their first day at school to provide to the health office a valid physical and up to date immunization record. Records will be requested from your child’s previous school, but it is the responsibility of the parent to ensure the documents are received by the school within 30 days.

Immunization Requirements for Preschool:

Diptheria, Tetanus, Pertussis - Three doses by 1 year of age. One additonal dose by 2nd birthday
Polio - Two doses by 1 year of age. One additional dose by 2nd birthday. Three doses for any child 24 months of age or older appropriately spaced.
Measles, Mumps, Rubella - One dose on or after the 1st birthday but prior to 24 months of age.
Haemophilus influenzai type b - Per your doctor's schedule
Hepatitis B - Three doses for all children 2 years of age or older.
Varicella - Two doses on or after the 1st birthday
Pneumococcal - Children 24 - 59 months of age who have not received the primary series of pneumococcal conjucate vaccine, according to the recommended vaccination schedule, shall show proof of receiving one dose of pneumococcal vaccine.

Immunization Requirements for First Entry into School - Kindergarten or First Grade:

Diptheria, Tetanus, Pertussis - Four or more doses of DTP/DTaP with last dose qualifying as a booster and received on or after the 4th birthday.
Polio - Four or more doses of the same type of Polio vaccine appropriately spaced with the last dose received on or after the 4th birthday.
Measles, Mumps, Rubella - 2 doses of Measles, Mumps, Rubella Vaccine, the 1st dose must have been received on or after the 1st birthday and the second dose no less than 28 days later.
Varicella - Students entering kindergarten must show proof of having received two doses of varicella vaccine, the first dose on or after the first birthday and the second dose no less than four weeks after the first dose.
Hepatitis B - Students entering kindergarten must have three doses administered at proper intervals.

Immunization Requirements for Students Entering Other Elementary Grades:

Diptheria, Tetanus, Pertussis - Three or more doses of DTP/DtaP or Td with the last dose received on or after the 4th birthday. Td booster every 10 years thereafter.
Polio - Four or more doses of Polio appropriately spaced with the last dose received on or after the 4th birthday.
Measles - 2 doses of Measles, Mumps and Rubella Vaccine, the 1st dose received on or after the 1st birthday, and the second dose no less than 28 days later.
Hepatitis B - Students entering 6th grade must have three doses administered at proper intervals.
Varicella - One dose on or after the 1st birthday with a second dose administered no earlier than 28 days later.

Immunization Requirements for 6th Grade:

Diptheria, Tetanus, Pertussis - Three or more doses of DTP/DtaP or Td with the last dose received on or after the 4th birthday.
Tdap - One dose recieved within the year prior to entering 6th grade.
Polio - Four or more doses of Polio appropriately spaced with the last dose received on or after the 4th birthday.
Measles - 2 doses of Measles, Mumps and Rubella Vaccine, the 1st dose received on or after the 1st birthday, and the second dose no less than 28 days later.
Hepatitis B - Students entering 6th grade must have three doses administered at proper intervals.
Varicella - One dose on or after the 1st birthday with a second dose administered no earlier than 28 days later.
Meningococcal - One dose of an immunization containing meningococcal conjucate vaccine (MCV4) is required for 6th grade entrance.
