• Food allergies or other health concerns (no snacks half day programs)

• Medications at school

Students are REQUIRED to have the following:

• School Physical with Required Immunizations on State of Illinois Form
  
  DUE BY AUGUST 1, 2025 in order to meet first day exclusion deadline

• Dental Exam on State of Illinois Form
  
  DUE BY MAY 15, 2026

• Eye Exam on State of Illinois Form
